I have seen a scaffold collapse once, at a maintenance site.
The shelf was placed on the third floor and workers were working on it when suddenly the entire shelf tilted to one side, causing two people to fall and one person to be seriously injured.

After the investigation, it was found that the reason was not “profound”: first, the shelf was not made to connect the wall; second, there were two upright poles without base plates, which were directly supported on the loose soil; third, the scaffold board was only half paved, but people went up.
Looking at each link individually, it is unlikely to cause any problems; But when they are stacked together, it becomes inevitable.
